Family Shade Healthcare Transitions Symposium

???? Featured Session at the Healthcare Transitions Symposium ????
 Healthcare transition for youth with disabilities is the planned move from child-centered to adult-centered care.
This transition helps young people build independence in managing their health, while ensuring they don’t experience gaps in care as they age into adulthood.
 The process includes:
✔️ Learning self-advocacy
✔️ Understanding legal rights
✔️ Preparing for an adult model of care
 For youth with intellectual and developmental disabilities (IDD), these steps are especially critical due to the unique challenges they face.
 ???? Join us at the 2025 Healthcare Transitions Symposium to explore strategies, resources, and real stories that make this process smoother for youth, families, and providers.
